원위부 금속 날개를 파손하지 않고도 30년 된 Brooker-Wil ls 경골 금속정을 성공적으로 제거한 경험: 증례보고

골수강내 금속정은 경골 간부 골절에서 가장 많이 사용되는 방법으로, 근위부 및 원위부 교합나사를 통해 회전 변형을 예방하도록만들어졌다. 이 때 원위부 나사 삽입 시 어려움을 겪게 되는데, 이러한 문제를 해결하고자 금속 날개를 펴는 방식을 통해 회전을막는 Brooker-Wills 경골 골수정이 개발되었다. 그러나 추후에 골수염 등의 이유로 Brooker-Wills 경골 골수정 제거를 시도할 때, 금속 날개가 접히지 않아 실패하는 경우가 많다. 이러한 이유로 금속정 제거 자체를 시도하지 않는 경우도 있었다. 저자들은 금속정 주위 감염으로 인해 내고정물 제거술이 필요했던 72세 환자에 대해 30년 된 Brooker-Wills 골수정을 날개의 파손 없이 제거한증례를 보고하고자 한다. An interlocking intramedullary nail is the most popular device for managing tibial shaft fractures. Interlocking screws should be inserted to prevent rotation at the fracture site, but inserting distal interlocking screws by free hand technique requires time and skill. The Brooker- Wills tibial nail was designed to stretch out distal metal fins after settling the nail in position. On the other hand, Brooker-Wills nails should be removed for reasons such as chronic osteomyelitis, and folding the distal metal fins is necessary. Nevertheless, many problems arise in this process. Usually, the removal of Brooker-Wills tibial nails is not attempted because of this trouble. The authors tried a new method and could fold the fins successfully in a 30-year-old Brooker-Wills tibial nail, which had to be removed to cure a chronic osteomyelitis around the nail. This paper reports the case of a 72-year-old male with a review of the relevant literature to suggest a useful method. KCI Citation Count: 0
