소아에서 포스너-슐로스만 증후군과 망막 동맥 만곡증이 동반된 1예

목적: 소아에서 포스너-슐로스만 증후군과 망막 동맥 만곡증이 동반되어 발생한 1예를 경험하였기에 이를 보고하고자 한다. 증례요약: 15세 남자 환아가 좌안 달무리 증상으로 내원하였다. 좌안 최대 교정 시력 0.6, 안압 59 mmHg로 측정되었고 세극등 현미경 검사상 좌안 각막 부종, 각막후면 침착물, 1+의 전방 세포, 깊은 전방 및 개방각 관찰되었고 안저검사상 좌안 시신경 유두함몰이 증가되고 좌안 망막 동맥 만곡증 소견보였다. 안압 강하 점안제, 스테로이드 점안제, NSAID 점안제 치료 후 안압 15 mmHg 측정되었으며 시신경 유두함몰이 감소되었다. 결론: 소아에서 고안압을 보일 경우 포스너-슐로스만 증후군도 감별진단으로 생각해야 하며 본 증례에서 동반된 망막 동맥 만곡증과의 연관성에 대해서는 향후 연구가 이루어져야 할 것이다. Purpose: To report a case of Posner-Schlossman syndrome with retinal arterial tortuosity in a young male. Case summary: A 15-year-old male presented to our department with a halo in his left vision. Best corrected visual acuity was 0.6 and intraocular pressure was 59 mm Hg in the left eye. Slit lamp examination revealed corneal edema, keratic precipitate, deep anterior chamber, anterior chamber cell (1+) and open angle. Fundus examination revealed increased cup/disc (C/D) ratio and retinal arterial tortuosity. After treatment with anti-glaucomatic eyedrops, steroid eyedrops, and nonsteroidal anti inflammatory drug (NSAID) eyedrops, intraocular pressure decreased to 15 mm Hg and C/D ratio decreased. Conclusions: Posner-Schlossman syndrome can occur even at an early age and must be included in the differential diagnosis of glaucoma in pediatric patients. Additional studies should be conducted regarding the association between retinal arterial tortuosity and Posner-Schlossman syndrome. J Korean Ophthalmol Soc 2014;55(11):1710-1713
