맥락막신생혈관의 광역학치료후 발생한 장액성 망막박리

목적: 맥락막신생혈관의 광역학치료후 발생하는 장액성 망막박리에 관하여 알아보고자 하였다. 대상과 방법: 맥락막신생혈관으로 광역학치료를 시행받고 치료후 2일째 장액성 망막박리 소견이 관찰된 환자 5명 6안을 대상으로 광역학치료 직전, 치료후 2일, 치료후 7일, 치료후 3주째에 광간섭단층촬영을 시행하여 망막의 두께를 측정하였다. 또한 환자들의 광역학치료의 시행횟수와 치료전 병변의 크기를 조사하였다. 결과: 광역학치료후 장액성 망막박리 소견이 관찰된 환자군의 중심망막두께는 광역학치료전 평균 313.2 μm에서 치료후 2일째 640.7 μm로 통계적으로 유의하게 증가하였고(P<0.01), 치료후 7일째 303.2 μm, 치료후 3주째 223.4 μm로 감소하였다. 환자들의 평균 광역학치료의 시행횟수는 평균 2.0회(범위: 1회~3회)였고, 치료전 병변의 크기는 평균 3122.8±1275.9 μm였다. 결론: 맥락막신생혈관의 광역학치료후 장액성 망막박리가 생길 수 있고, 이는 별다른 치료없이 광역학치료후 7일째에 소실됨을 알 수 있었다. <한안지 48(10):1354-1361, 2007> Purpose: To quantify the development and resolution of serous retinal detachment in patients with choroidal neovascularization (CNV) following photodynamic therapy (PDT). Methods: Six eyes of five patients who developed serous retinal detachment two days after PDT were included in this study. Retinal thickness was measured by optical coherence tomography (OCT) before PDT, and at two days, seven days, and three weeks after PDT. The number of PDT and the greatest linear dimension (GLD) of the CNV lesion were recorded. Results: Serous retinal detachment was demonstrated on OCT at two days after PDT. Retinal elevation increased significantly from 313.2 μm before PDT to 640.7 μm two days after PDT (P<0.01); elevation and decreased to 303.2 μm seven days after PDT, and decreased to 223.4 μm three weeks after PDT. The mean number of PDT treatments 2.0 (range: 1~3), and the mean GLD before PDT was 3122.8±1275.9 μm. Conclusions: This study suggest that serous retinal detachment in patients with CNV may develop following PDT but may also resolve spontaneously seven days after PDT.
