병행 하사근 약화술이 간헐외사시의 외직근 후전술의 결과에 미치는 영향

목적: 간헐외사시와 동반된 하사근 기능항진증을 동시에 수술 교정해야 하는 경우, 하사근 약화술이 외직근 수술 효과에 어떤 영향을 주는지 관찰함으로써 외직근과 하사근의 병행 수술 시의 외직근 수술량 결정에 참고하고자 한다. 대상과 방법: 간헐외사시에 대하여 양안 외직근 후전술을 단독으로 받은 환자군(1군)과 간헐외사시와 동반된 하사근 기능항진증으로 양안 외직근 후전술과 양안 하사근 약화술을 받은 환자군(2군)을 대상으로, 외직근 수술량에 따른 수술 전, 수술 후 3개월, 12개월의 원거리 편위각과 편위각의 수술 전후 변화량을 비교하였다. 결과: 1군(98명)과 2군(77명) 사이에 수술 전 편위각은 통계학적 차이가 없었으며, 수술 전 인자도 차이가 없었다( p＞0.05). 1군과 2군사이에 외직근 수술량에 따른 수술 전, 수술 후 3개월, 12개월에서의 편위각과 편위각의 수술 전후 변화량은 통계학적 차이가 없었다( p＞0.05). 결론: 하사근 약화술은 외직근 후전술 후 수평편위각에 유의한 변화를 주지 않으므로, 외직근 후전술과 하사근 약화술 병행시행 시단독 외직근 후전술에 비해 특별히 수술량에 대한 보정은 필요하지 않을 것으로 생각한다.<대한안과학회지 2015;56(2):249-253> Purpose: To investigate whether combining inferior oblique weakening procedures with bilateral lateral rectus recession as a surgical approach to intermittent exotropia enhances postoperative horizontal angle of deviation over bilateral lateral rectus recession alone. Methods: Patients were allocated to one of two surgical groups: a bilateral lateral rectus recession group (Group 1) or a bilateral lateral rectus recession with bilateral inferior oblique weakening group (Group 2). Group differences in the angle of distance deviation were evaluated preoperatively and at 3- and 12-months postoperatively. Results: There were no significant differences in preoperative factors or angles of deviation between Group 1 (n = 98) and Group 2 (n = 77); no significant intergroup difference in the angle of deviation was observed at any postoperative time point (p > 0.05). Conclusions: Combined inferior oblique weakening procedures did not significantly influence the horizontal angle of deviation after surgery for intermittent exotropia.
