잔류된 금속성 안와 이물 제거 1례

안와조직의 관통상 후 간과된 안와내 이물은 주변조직에 영향을 미쳐 시력감소, 안구 운동장애, 복시, 사시, 안와 봉와직염, 안와농양 등 여러 안과적 문제를 야기하고 인접한 중추신경계를 침범하는 경우 뇌막염, 뇌농양 같은 치명적인 합병증을 초래할 수 있다. 그러므로 안와이물을 조기에 정확히 진단하고 적절히 치료하여 합병증을 예방하고 손상된 조지을 해부학적, 기능적으로 복구하여야겠다. 저자들은 하안검을 관통한 안외상 후 5개월간 안와내 이물이 간과되었던 환자가 안와종창과 안구통으로 내원하여 컴퓨터 안와 단층촬영으로 안와이물을 확인하고 외측 안와절개술(lateral orbitomy)를 이용하여 금속성의 안와이물을 제거한 1례를 경험하였다(한안지 40:1127~1132, 1999). Retained intraorbital foreign body after penetrating orbital injury may affect adjacent tissue and lead to many ophthalmological sequeles such as visual disturbance, EOM limitation, diplopia, strabismus, orbital cellulitis and orbital abscess. If intraorbital foreign body intrudes into adjacent CNS system, it may lead to fatal complications such as meningitis and brain abscess. We should diagnose and treat intraorbital foreig body early and accurately to prevent complications, and rehabilitate damaged tissue anatomically and functionally. A patient with retained intraorbital foreign body after penetrating lower lid injury was transferred to our hospital for lid swelling and periocular pain. We diagnosed and confirmed intraorbital foreign body with use of plain X ray and orbital CT, and removed it with lateral orbitotomy surgical approach(J Korean Ophthalmol Soc 40:1127~1132, 1999).
