인슐린 비의존형 당뇨환자에서의 혈장 PAI - 1 항원농도와 당뇨망막병증의 상관관계

Summary: | 인슐린 비의존형 당뇨환자에서 혈장 Plasminogen activator inhibitor-1(PAI-1)항원 농도와 당뇨망막병증사이의 상관관계를 알아보고자 하였다. 89명의 인슐린 비의존형 당뇨환자 (평균연령 59.8 ±11.3세)와 25명의 정상인 대조군(52.8 ±14.7세)을 대상으로 혈장 PAI-1 항원농도를 비교측정하였다. 당뇨환자군은 망막병증의 정도에 따라 정상(34명),비증식당뇨망막병증(29명),증식당뇨망막병증(26명)의 군으로 분류하였다. PAI-1 항원 농도의 측정은 효소면역학적검정법(enzyme immunoassay)을 사용하였다. PAI-1 항원농도는 당뇨환자군(34.56 ±17.80ng/㎖)이 정상대조군(20.35 ±15.78ng/㎖)에 비해 통계적으로 유의하게 높았다(p<0.05). 당뇨환자군내에서는 증식당뇨망막병증군(27.39 ±15.54ng/㎖)이 비증식당뇨망막병증군(39.43 ±20.17ng/㎖)이나 당뇨망막병증 정상군(36.87 ±23.31ng/㎖)에 비해 통계적으로 유의하게 낮은 PAI-1 항원농도를 보였는데 이는 광범위한 순환계의 혈관내피세포 손상으로 인한 것으로 사료된다(p<0.05). 본 연구의 결과는 PAI-1 항원농도의 증가로 인한 섬유소 용해기전의 손상이 망막병증의 발생과 연관성이 있다는 학설을 뒷받침하며 따라서 혈장 PAI-1 항원 농도의 측정은 당뇨망막병증의 발생,특히 증식당뇨망막병증의 발생 및 진행에 유용한 지표가 될 수 있으리라 사료된다(한안지41:127~132,2000).
To examine the relationship between plasma plasminogen activator inhibitor-1(PAI-1)antigen concentration and diabetic retinopathy in non-insulin dependent diabetic patients,PAI-1 antigen levels and some fibrinolytic parameters were studied in 89 non-insulin dependent diabetic patients[mean age 59.8 ±11.3 years]and 25 normal adults as control(meanage 52.8 ±14.7 years). The diabetic patients were classified as three subgroups: no DR(n=34),NPDR(n=29)and PDR(n=26)according to the degree of retinopathy. The PAI-1 antigen concentration was measured by enzyme immunoassay(Innotest PAI Ag kit).The diabetic patients had a significantly higher mean PAI-1 antigen level (34.56 ±17.80ng/㎖)compared to a control group(20.35 ±15.78ng/㎖)(p<0.05).Plasma PAI-1 antigen level was significantly lower in diabetic patients with PDR(27.39 ±15.54ng/㎖)than in diabetics with no DR(36.87 ±23.31ng /㎖)or NPDR(39.43 ±2 0.17ng/㎖)(p<0.05), probably because of more extensive systemic endothelial damage. These results support the hypothesis that impaired fibrinolysis due to elevated PAI-1 is associated with the development of retinopathy, and therefore the levels of PAI-1 can be used as useful indicator for the development and progression of proliferative retinopathy(J Korean Ophthalmol Soc41:127~132,2000). |
