황반전출혈에서 Nd : YAG 레이저를 이용한 광파괴술

황반전출혈(망막전출혈,유리체하출혈,내경계막하출혈)은 갑작스런 중심시력의 장애를 초래하는 질환으로 여러 선행질환이 있는 경우에서 발생한다. 출혈이 오래 지속되는 경우에는 영구적인 시력저하를 초래하는 합병증이 발생하게 된다. 황반전출혈에 대한 치료로는 보존적인 치료방법과 레이저 광파괴술,조기 유리체 절제술이 제안되었다. 이 중 레이저 광파괴술은 출혈의 흡수가 지연되는 경우나 직업상 양안시기능이 시급한 경우,반대측 안의 시력이 좋지 않은 경우에서와 같이 일반적으로 빠른 효과가 필요한 경우에서 시행할 수 있다. 저자들은 4명의 황반전출혈...

Premacular hemorrhage(preretinal,subhyaloid,or sub-internal limiting membrane hemorrhage)impairs central vision abruptly and occurs in various underlying diseases.In cases of persistent hemorrhage,permanent impairment of vision may occur. Proposed therapeutic options for premacular hemorrhage consist of conservative treatment,laser photodisruption and early vitrectomy.Laser photodisruption may be performed in cases of persistent or slow-clearing hemorrhage,in patients with occupational need for prompt restoration of binocular vision or in patients with poor vision in their fellow eyes.The authors report 4 patients with premacular hemorrhage who were treated with Nd:YAG laser photodisruption with the review of the literatures(J Korean Ophthalmol Soc 40:2020~2026,1999).
