클로자핀 복용으로 유발된 각막 및 수정체 색소침착 1예

목적: 장기간 클로자핀을 복용한 환자에서 각막과 수정체의 색소침착 1예를 보고하고자 한다. 증례요약: 조현병을 앓고 있는 56세 남자가 양안의 시력저하를 주소로 내원하였다. 환자는 5년간 매일 클로자핀 200 mg, 로라제팜 1 mg을 복용하였다. 첫 내원 시 양안 최대 교정시력은 양안 20/32였다. 세극등검사에서 양안 각막내피세포층에 밝고 미세한 회갈색의 침착물이 관찰되었고, 산동 후 수정체 전낭아래 방사선상의 색소침착이 중앙부에 관찰되었다. 결론: 클로자핀의 복용은 각막과 수정체에 색소침착을 유발하며, 시력저하를 일으킨다. 앞...

Purpose: To report a case of corneal and lenticular pigmentation after prolonged clozapine therapy. Case summary: A 56-year-old male visited our hospital with a progressive decline in vision that affected both eyes. He had a history of schizophrenia. He was being treated with 200 mg clozapine and 1 mg lorazepam daily, and had been treated with clozapine for 5 years. At the first visit, his best-corrected-visual acuity was 20/32 in both eyes. Slit lamp examination of the corneas showed bright, fine, grayish-brown deposits on the endothelium, and on dilation, bilateral central stellate opacity of the anterior portion of the lens capsule was revealed. Conclusions: Clozapine may induce corneal and lenticular pigmentation and thus may lead to a decline in vision. Patients on long-term clozapine therapy should be considered for regular ophthalmic review. KCI Citation Count: 0
