백색증 환자에서 망막박리 수술 1예

목적 : 안피부형 백색증 환자의 망막박리 수술 1예를 경험하여 수술 시 주의점 등을 문헌 고찰과 함께 보고하고자 한다. 증례요약 : 44세 남자 환자가 3일 전 갑자기 발생한 좌안의 시력 저하를 주소로 내원하였다. 초진 시 좌안 시력은 안전수동으로 안저검사상 황반부를 포함한 상이측의 망막박리가 발견되었다. 내원 다음날 평면부유리체절제술을 시행하였으며 액체공기교환술 후 망막유착을 위한 눈속레이저광응고술을 시행하였으나 레이저에 의한 반흔이 생기지 않아 망막-맥락막 유착을 기대할 수 없어 냉동유착술을 이용하여 박리된 망막을 유착시켰고, 실리콘기름을 주입하였다. 수술 후 10개월째 망막은 잘 유착되어 있으며 증식유리체망막병증 등 기타 합병증은 관찰되지 않고 있다. 결론 : 백색증 환자는 멜라닌 색소가 결핍되어 있어 망막박리 수술 시 통상적인 레이저 광응고술에 의한 망막유착 효과를 기대할 수 없기 때문에 냉동유착술이 필요함을 보고하는 바이다. Purpose: To report a case of retinal detachment surgery in a patient with oculocutaneous albinism. Case summary: A 44-year-old man visited our clinic complaining of decreased visual acuity in his left eye. His best corrected visual acuity was hand movement in his left eye, and rhegmatogenous retinal detachment involving the macula at the superior temporal site was found. We performed pars plana vitrectomy and attempted to reattach the retina using endolaser photocoagulation; however, the laser burn was not made, and we failed to reattach the retina. At that point, we carried out cryopexy around the retinal tear, and injected silicone oil into the vitreous cavity. Ten months after surgery, his best corrected visual acuity was 0.06, and there was no recurrent retinal detachment or proliferative vitreoretinopathy. Conclusions: In patients with albinism with melanin deficiency, cryopexy is more useful than laser photocoagulation for retinal detachment surgery. KCI Citation Count: 0
