ITST (Intertrochanteric/Subtrochanteric) 골수강내 정을 이용한 대퇴 전자부 골절의 치료

목적: ITST (Intertrochanteric/Subtrochanteric) 골수강내 정을 사용한 대퇴 전자부 골절의 수술적 치료에 대해 임상적 및 방사선적 결과를 평가해 보고자 하였다. 대상 및 방법: 2006년 1월부터 2007년 5월까지 ITST 골수강내 정으로 대퇴 전자부 골절을 치료받고, 12개월 이상 추시 관찰이 가능했던 40예를 대상으로 하였다. 남자가 13예, 여자가 27예였으며, 평균 연령은 75.6세였다. 임상적 결과는 Ceder의 보행 평가를 이용하여 평가하였고, 방사선적 결과는 대퇴골 경간각의 변화 및 지연 나사의 활강 정도를 이용하여 평가하였다. 결과: 골 유합 시기는 평균 13.5주였다. 수술 후 보행 능력은 수술 전 보행 능력을 회복한 경우를 만족으로 하여 34예 (85%)에서 만족할 만한 결과를 얻었으며, 최종 추시상 대퇴골 경간각의 변화는 평균 5.21o, 지연 나사의 활강은 평균 5.78 mm였다. 결론: ITST 골수강내 정은 대퇴 전자부 골절의 치료 시 다른 내고정 장치에 비해 합병증이 적고, 비교적 양호한 임상적 및 방사선적 치료 결과를 보였다. Purpose: To evaluate the clinical and radiographic results of treatment of trochanteric fracture with ITST (Intertrochanteric/ Subtrochanteric) nail. Materials and Methods: We reviewed the results of 40 cases of trochanteric fracture treated with ITST from January 2006 to May 2007, which could be followed up for more than 12 months. The cases include 13 males and 27 females, and the mean age is 75.6 years old. The clinical results were evaluated by Ceder mobility assessment, and the radiographic results were evaluated by the change of femoral neck-shaft angle and sliding of lag screw. Results: The mean bone union time is 13.5 weeks. Thirty four cases (85%) were recovered to pre-injury state of walking ability. The change of neck-shaft angle was an average of 5.21o and the sliding distance of lag screw was an average of 5.78 mm. Complications were occurred in 4 patients (10%). Conclusion: The ITST nail were seen good results in treatment of trochanteric fracture and has relatively less complications than other internal fixator. KCI Citation Count: 1
