Summary:A fishbone foreign body in the hypopharynx or esophagus is a typical emergency requiring urgent removal. On the other hand, foreign bodies in the extra-pharyngeal space are relatively rare, but often are difficult to remove. We have experienced four cases of an extra-pharyngeal fishbone foreign body in the past 20 years. In two cases, the foreign bodies were removed by pharyngeal mucosal incision and intraluminal manipulation, and in the other two cases they were removed by an external cervical incision of the neck. Preoperative CT was useful for determination of the surgical approach to remove the foreign bodies, but in some cases it was difficult to identify a foreign body during the operation. Especially in foreign body excision by pharyngeal manipulation, preoperative 3D-CT was more useful.
