Summary:This study examined the correlation between cardiac load indicators and the time taken for a self-bed bath in order to develop a safer self-bed bath method and familiarize cardiac-disease patients with self-care. The subjects were 10 healthy adult males, who performed a self-bed bath of both the upper extremities and the thoracic-abdominal part while sitting on a bed vertically inclined at 60°. Pulse, blood pressure, and oxygen intake (VO2) were measured before, during, and after the self-bed bath to calculate myocardial oxygen consumption (DP). The reciprocating self-bed bath motion was repeated five or ten times at 50 times/min. The results showed that the values for pulse, DP, and VO2 (with time) significantly increased, suggesting that cardiac load might increase even with light motion. Cardiorespiratory dynamics and metabolism increased during the motion. Thus, to enable cardiac-disease patients to take a self-bed bath safely, a caregiver should observe the cardiac load on the patient based on the measured pulse and should consider the time taken for self-care.  本研究は, 心疾患患者のセルフケアの拡大に向けてより安全な自己清拭動作の方法を開発するため, 心臓の負荷状態と自己清拭動作の継続時間との関係を明らかにすることを目的として行った. 実験は, 健康な成人男性10名を対象とし, 60度のベッド拳上座位で両上肢・胸腹部位の自己清拭動作を行い, 実施前・中・後を通して心拍数・血圧・分時酸素摂取量 (VO2) を測定し, 心筋酸素消費量 (DP) を算出した. 各部位を拭く回数は往復5回と往復10回とし, 50回/分の速さで実施した. その結果, 動作中に心拍数・DP・VO2は有意に増加を認め, VO2においては継続時間に伴い増加傾向を示し, 低強度の活動であっても継続時間に伴い心臓への負荷が大きくなることが明らかとなった. さらに, 動作中は呼吸循環動態や代謝が高まることから, 心疾患患者への安全な自己清拭の実施において, 心拍数を目安に心臓への負荷状態を考慮するとともにケアの継続時間を検討する必要性が示唆された.
