Summary:The patient was a 74-year-old man who was detected to have elongation of the left hepatic duct by screening abdominal ultrasonography. More detailed examination by MRCP and ERCP revealed constriction of the left hepatic duct. A definitive diagnosis could not be made by biopsy, however, under the suspected diagnosis of cholangiocarcinoma of the lateral lobe, we performed resection of the left lobe of the liver. Histopathology showed IgG4-related inflammatory pseudotumor. IgG4-related inflammatory pseudotumor is a relatively rare disease that can occur in any organ of the body. In the hepatobiliary region, it is known to be significantly associated with autoimmune pancreatitis and sclerosing cholangitis. We encountered a case of IgG4-related inflammatory pseudotumor of the liver, and report this case with a review of the literature.
