Summary:Awake intubation is very stressful for both patients and anesthesiologists because of several difficulties. It depends on the experience and skill of anesthesiologists. Here, we describe the procedure of awake intubation under inhalation of 4% lidocaine and AirwayScope on four volunteer anesthesiologists, and report a putative difficult airway case. Inhalation of local anesthetic alone could not sufficiently suppress the airway reflex in the patient. The procedure together with the administration of fentanyl up to 300 microg achieved successful and safe intubation. This method would be useful for awake intubation in cooperating patients.
