Summary:  According to EAU, AUA, and JUA guideline, percutaneous nephrolithotomy (PCNL) has been still gold standard for greater than 2 cm kidney stone. With technological development, endoscopic combined intrarenal surgery (ECIRS) which is approached from simultaneously retrograde and percutaneous side has gradually grown up in our country (Japan). However, both PCNL and ECIRS are based on percutaneous access into renal collecting system, which is required higher technical skill. Based on these backgrounds, we want to describe step by step our technique named by well-managed approach for less bleeding in this term.
