Summary:The incidence of intracranial acute subdural hematoma after spinal anesthesia is very rare(1/500,000-1/1,000,000), but it is a complication that can cause severe neurological symptoms and sequelae. The mechanism of occurrence of intracranial acute subdural hematoma is thought to be caused by the leakage of spinal fluid from puncture as well as post-dural puncture headaches, and the similarities of symptoms often make it difficult to distinguish between them. In this study, we show a case in which the patient who had a headache on the second day after surgery after selective caesarean section with spinal anesthesia for twin pregnancy and we were diagnosed with intracranial acute subdural hematoma by a simple CT examination. It is a rare but serious complication after spinal anesthesia. So it is necessary to treat the patient with intracranial acute subdural hematoma in mind.
