Summary:The frequency and clinical significance of expression of the hematopoietic stem cell-associated antigen CD34 in leukemia cells were assessed in 80 cases of acute lymphoblastic leukemia (ALL) and 75 cases of acute myeloblastic leukemia (AML). The CD34 antigen was present in 71.3% of ALL and 53.3% of AML patients. Comparing 23 CD34-ALL cases with 57 CD34+ALL cases for expression of other antigens, we found that CD34+ ALL patients were more likely to have blasts coexpressing myeloid-associated antigens CD13 and CD33. There was no significant difference in clinical characteristics between CD34+ and CD34-ALL patients. CD34 expres-sion in AML patients was found to have a strong relationship with subtypes of FAB classification. 80% of M1 and M2 patients were CD34 positive while only 25.9% of M3, M4 and M5 were CD34 positive. CD34+myeloblasts were more likely to coexpress the lymphocyte-associated antigens CD2, CD10 and CD19. In this study no prognostic significance of childhood ALL and AML CD34 expression was found. 急性リンパ性白血病80例および急性骨髄性白血病75例について, 白血病細胞の造血幹細胞関連抗原CD34発現の頻度と臨床意義について検討した.ALLでは71.3%, AMLでは53.3%がCD34抗原が陽性であった.CD34陰性ALL23例とCD34陽性ALL57例について, 他の抗原発現について比較すると, CD34陽性AUでは穎粒球関連抗原であるCDl3とCD33を同時に発現している傾向があった.CD34陰性ALLとCD34陽性ALLの間では臨床特徴について有意な違いは見られなかった.AMLにおけるCD34発現はFAB分類の病型と強い関連性があった.M1, M2の80%がCD34陽性であったのに対し, M3, M4およびM5は25.9%のみCD34陽性であった.CD34陽性の骨髄芽球はリンパ球関連抗原であるCD2, CD10やCD19も同時に発現する傾向があった.この研究では, ALLとAMLにおけるCD34発現の予後因子としての意義を認めなかった.
