An Adult Case of Intussuception due to Ileal Lipoma Diagnosed by Preoperative Colonoscopy

We report a case of intussusception in adult due to ileal lipoma diagnosed by colonoscopic examination. A 49-year-old female was referred to our hospital with complaints of lower abdominal pain and bloody stool. An emergency colonoscopy revealed an intussusception due to ileal tumor. Endosonography and abdominal CT suggest a lipoma of the terminal ileum, and histological examination of biopsy specimens confirmed the findings. This case suggest that the colonoscopy would be useful for preoperative diagnosis of intussusception of the terminal ileum. 今回我々は内視鏡にて術前診断し得た回腸脂肪腫による成人腸重積症の1例を経験したので若干の考察を含め報告する. 症例は49歳女性. 腹痛, 下血にて来院し緊急下部内視鏡検査にて腸重積の所見を認め, 生検組織, 超音波内視鏡検査, 腹部CTなどより脂肪腫と診断し, 回盲部切除術施行. 病理組織診断も脂肪腫であった. 本症例より腸重積症に対して, (1)組織診断が得られる, (2)出血などの病変の状態が把握できる, (3)重積解除により質的診断を試みる時間を稼げるなどから大腸内視鏡検査は有用であった.
