Summary: | Background. Patients with keloidal diathesis are prone to subglottic stenosis after endotracheal intubation. Stenoses close to the glottis increase the risk of difficult airway. Case. A 70-year-old woman with keloidal diathesis was referred to our hospital because of dyspnea and hoarseness upon exertion that had been worsening over the past 2 years. The patient had undergone 9 previous surgical interventions under general anesthesia in the past 10 years for a uterine prolapse and an eyelid tumor. She was diagnosed with subglottic stenosis caused by scarring of the endotracheal granuloma. We performed tracheal resection and cricotracheal anastomosis with bronchoscopy under anesthesia using a laryngeal mask, which allowed for the confirmation of the extent of tracheal resection by bronchoscopy. The patient was discharged five days after surgery without any major postoperative complications. No restenosis has been noted in the year following surgery. Conclusion. We successfully performed radical surgery for subglottic tracheal stenosis in a patient with keloidal diathesis using intraoperative bronchoscopy in combination with a laryngeal mask. |
