AbstractList 「要旨」 抗凝固療法を受けている患者に対する術後疼痛管理に際しては, 血栓性疾患の病態, 抗凝固療法に関する詳細な情報を手に入れ, 抗凝固療法を中止・継続することの功罪を十分考慮した上で周術期の治療指針を決定する. 上腹部手術の術後痛は非常に強いため, さまぎまな鎮痛手段を, 患者の状態, 手術, 麻酔法, 術後経過などのcontextに応じて効果的に用いる必要がある. その中でも区域麻酔は抗凝固療法の有無にかかわらず, 鎮静や呼吸抑制などの副作用を起こすことなく優れた鎮痛効果をもたらすことが可能であり, 大きな武器になると考えられる. 「はじめに」 Context-sensitive analgesiaとは, 2008年にFanelliら1)が提唱した概念である. 彼らは従来から推奨されているmultimodal analgesiaをさらに発展させ, patient context, surgical context, anesthesia context, postoperative contextという4つのcontext(=文脈, 前後関係)に加え, 周術期における痛み・痛覚過敏の発生要因の時間的推移を分子生物学的レベルまで考慮し, それぞれの状況に応じた鎮痛法を選択することによって, より完全な術後鎮痛を達成することができるのではないかと主張している.
