側頭骨に生じたGiant Cell Reparative Granulomaの1症例
巨細胞修復性肉芽腫（Giant Cell Reparative Granuloma: GCRG）は上顎骨，下顎骨に生じることが多く，側頭骨に発生する報告は少ない。今回われわれは側頭骨を原発としたGiant Cell Reparative Granulomaに対して手術的加療を行い，完全切除を得た症例を経験した。症例は47歳女性で，頭痛，左耳痛を主訴に当科を紹介受診した。CTにて左側頭骨に腫瘤性病変を認め，鼓室内，中頭蓋底，顎関節窩への進展が疑われた。中耳からの生検で巨細胞肉芽腫の診断となった。外側側頭骨切除術，中頭蓋底合併切除術，顎関節合併切除術を行い，腫瘍を全摘出した。硬膜欠損部位は側頭筋膜...

| Abstract | 巨細胞修復性肉芽腫（Giant Cell Reparative Granuloma: GCRG）は上顎骨，下顎骨に生じることが多く，側頭骨に発生する報告は少ない。今回われわれは側頭骨を原発としたGiant Cell Reparative Granulomaに対して手術的加療を行い，完全切除を得た症例を経験した。症例は47歳女性で，頭痛，左耳痛を主訴に当科を紹介受診した。CTにて左側頭骨に腫瘤性病変を認め，鼓室内，中頭蓋底，顎関節窩への進展が疑われた。中耳からの生検で巨細胞肉芽腫の診断となった。外側側頭骨切除術，中頭蓋底合併切除術，顎関節合併切除術を行い，腫瘍を全摘出した。硬膜欠損部位は側頭筋膜で再建し，顎関節・頬骨切除部位は肋骨付き前鋸筋皮弁で再建を行った。術後はミキサー食の摂食が良好であり，術後約2週間で軽快退院となった。外耳道閉鎖による伝音難聴を認めたが，顔面神経や内耳機能は温存された。Giant Cell Reparative Granulomaは全摘が困難であった場合の再発率が高く，可能な限り完全摘出が望ましいとされる。本症例では，脳神経外科，形成外科と合同で腫瘍全摘・再建術を行い，良好な術後経過をたどったので報告する。 |
