外傷性上腸間膜動静脈損傷の1例
症例は19歳の女性で，腹部鈍的外傷により腹腔内出血を来し緊急手術施行した。脾臓損傷，上腸間膜動静脈の出血を認めた。脾臓を摘出し，上腸間膜動静脈領域の出血部位は結紮およびガーゼによる圧迫止血によるダメージコントール手術を施行し閉腹した。ICUで循環動態改善後に血管造影検査を行ったところ，上腸間膜動脈は中結腸動脈分岐部より遠位で完全断裂しており，空腸第1枝は造影されなかった。腹部造影CTで空腸の造影不良部位を認めたため，初回手術より48時間後にセカンドルック手術を行った。トライツ靭帯から10cmより空腸の虚血壊死を認めた。壊死部の空腸を約120cm切除し小腸を機能的端々吻合した。術後，膵臓浮腫によ...
