Summary:Since the Smith-Petersen nail, many types of apparatus have been devised for internal fixation of femoral neck fracture. This fracrure usually occur in aged people, and they often accompany some general diseases. So small exposures are suitable in order to prevent general complications. Stable fixation which could give the early movement without pain is also needed to prevent general complications. But few apparatus satisfy both small exposures and stable fixations. We devised new protector using cannnulated cancellous hip screw made in Ace. By using this protector we can get both small exposures and stable fixation like screw. This protector has a large central tunnel for reception of head of cancellous hip screw. Procedures areas follows: 1) Under roentgenographic control, the skin cut is made about 2cm at lateral side of the thigh. 2) Stick the protector to the lateral shaft of the femur. 3) A guide pin is inserted into proximal fragment at an angle of 45° with the shaft of femur, and screw is driven in over a guide pin. 4) Same procedures are applied for other 2 screws. All procedures can be completed under local anesthesia. We applied this method to 17 poor risk aged patients with general complications, and performed operations safely under local anesthesia.
