十二指腸原発GISTの1例

われわれは潰瘍形成,出血を反復した十二指腸原発GISTの症例を経験した.症例は77歳,男性.上部消化管透視,内視鏡検査にて十二指腸第1部から第2部に潰瘍と高い周堤を伴う隆起性病変を認め,腹部CT,超音波検査にて十二指腸に腫瘤性病変を認めた.生検で間葉系悪性腫瘍が疑われ手術を施行,術式は膵頭十二指腸切除術を選択した.十二指腸第2部を中心に最大径9 cm大の充実性腫瘍を認め,病理組織学的には類円形核の紡錘形～多角形細胞が線維束状ないし充実性に増生し核分裂像は平均4.7個/50 HPFであった.免疫染色ではc-kit, CD34, vimentinが陽性, α-SMA, HHF-35, S-100で...
