耳下腺癌の病理
唾液腺腫瘍は比較的まれで、その約 80％は耳下腺に発生する。そして、耳下腺腫瘍の約 20％は悪性で、そのうちの大多数は癌腫である。耳下腺癌は、組織像が多彩な上、数多くの組織型や種々の亜型が存在しており、また、異なる組織型であっても部分的には類似した組織像がみられるため、一般病理医にとって診断に難渋することが少なくない。さらに、耳下腺癌には生物学的態度の異なる腫瘍群が混在している。耳下腺癌の組織分類は複雑であるが、組織型の判定は予後の予測や治療方針の決定に直結する。依然として HE 染色が病理診断の基本となるが、免疫組織化学は診断精度を高めたり、腫瘍細胞の性格や分化、細胞増殖能、および癌遺伝子の...
