Three adult cases of HBV-associated MPGN

Summary:We investigated three cases with HBV-associated glomerulonephritis (MPGN Type III, I, and I), two cases of which were hospitalized under a diagnosis of nephrotic syndrome and the other was admitted for chance proteinuria. Serum HBsAg and HBcAb were positive in all the cases ; HBeAg was positive in the two but was negative in the other in which it turned positive after three months. Although subepithelial dense deposits additionally to the mesangial and subendothelial ones were found in all the cases, direct immunofluorescence showed negative HBeAg but only granular positive HBsAg in the glomerulus. All the cases were diagnosed as MPGN but they were more pleomorphic in glomerular changes than ordinary idiopathic MPGN. These data indicated that adult HBV-associated MPGN seemed different to some extent in morphological as well as clinical aspects from the pediatric one, and that HBV-associated glomerulonephritis might show such a wide pathologic spectrum with the multisystem disorders as lupus nephritis.
