The position on record is specified in the “Standards of clinical nursing practice” in the Japanese Nursing Association as, “A series of procedures for clinical nursing practice is recorded”. The position on nursing records; the system, how nursing records should be written and so on, are covered in “Guidelines for the Maintenance of Nursing Records”.I made a path based on those criteria so that we can develop nursing procedures on path sheets, aiming for:* satisfaction of the conditions;*a balance with insurance medical care;* improvement to, and efficiency of, nursing records based on an individual patient's case.It is five years since we began using this path. Examining the path as a nursing record I found using a single paper record was inadequate, so too was the lack of patient response. I have therefore reviewed our hospital path from a nursing and administrative viewpoint. As a result it highlighted three problems as follows:* problems with the path sheet;* problems with staff education;* the level of care devoted to patients by nursing staff.We now have to address each of these issues. 日本看護協会の「看護業務基準」では記録の位置づけについて「看護実践の一連の過程は記録される。」と記されている。「看護記録の開示に関するガイドライン」に看護記録の位置づけ・システム・あり方などについて書かれている。クリティカルパス (以下、パス) 作成にあたりこれらにもとづき・看護記録の要件を満たしていること、保険診療とのかねあいや患者の個別性をふまえた記録の質の向上と効率化を目指し、パスシート上で看護過程の展開ができるように考えて作成した。このパスを使用し始めて5年目となった。看護の記録としてのパスを考えたとき、1枚の紙パスシートの限界と患者の反応のないものとなっているように感じたため、当院のパスを看護記録としての視点で振り返った。その結果、 (1) パスシートの問題 (2) スタッフ教育の問題 (3) 患者をみっある看護師自身、以上の3点の問題が明確になった。今後、これらの課題を一つ一つ解決していかなければならない。
