Summary:This is a report of obturator foramen hernia. An 81-year-old woman was admitted to the hospital because of abdominal pain. Howship-Romberg sign, an oppressed symptom of obturator nerve, was positive. A scout film of the abdomen confirmed small bowel obstruction. Pelvic Computed Tomography (CT) showed a round homogenous density mass between the pectineus and external obturator muscles. Under a diagnosis of obturator hernia, emergency operation was performed. A part of intestinal loop was strangulated into the left obturator canal. Reduction of the strangulated intestinal loop was done. The hernia sac was inverted, ligated and excised. Obturator foramen hernia is rare but possible cause of small bowel obstruction in the aged patients, especially in females. The resultant delay in diagnosis contributes to a high mortality rate. The use of CT as well as elicitation of Howship-Romberg sign is essential in the early diagnosis of the disease.
