Summary:A case of psoas abscess associated with diabetes mellitus is reported. A 63-year-old woman with diabetes mellitus, who had been treated with insulin at another hospital, noticed low back pain a month before presentation. Later, left back pain appeared and CT scan revealed an abnormal shadow in her left psoas muscle. She was referred to us for further evaluation and treatment. On admission, leukocytosis, increased serum C-reactive protein (CRP) and costo-vertebral angle tenderness were found. Despite intravenous administration of antibiotics, a fever rising as high as 39-40°C developed and CT scan showed swelling of the left psoas muscle and multiple low density areas in the muscle. The diagnosis of psoas abscess was made and surgical drainage was performed. Pus amounting to 600ml was drained. Streptococcus agalactiae was cultured from the pus and urine. After surgery, the fever subsided and CRP level and leukocytosis normalized.
