Summary: | A case of diabetes mellitus who presented an urachal abscess as an initial symptom is reported. A 62-year-old woman was admitted to our hospital for an evaluation of her lower abdominal pain and walking difficulty which had appeared several days before. On admission, a fever up to 39°C and tenderness on pubic symphysis were noted. Increased serum level of C-reacting protein (CRP) was found and her fasting blood glucose and hemoglobin A1C were, 158mg/dl and 8.4%, respectively. Staphylococcus aureus was detected from blood culture on admission. The CT scan of abdomen revealed a mass just below her pubic symphysis which showed a rim enhancement with contrast agent and contained a gas. A pus was obtained by puncture, then, a diagnosis of urachal abscess was confirmed. Her fever subsided and serum level of CRP became normal after the treatment of drainage and antibiotics. Neither renal nor retinal diabetic complication was found and her blood glucose were well controlled on diet therapy alone. There is only one report which presented the case of urachal abscess complicated with diabetes mellitus. Furthermore, this is the first report which presents the case of diabetes mellitus showing urachal abscess as the initial symptom. |
