Summary:We report a case of successful splenic autotransplantation following splenectomy in a child with blunt abdominal trauma. The growth of splenic tissue was evaluated by scintigraphic activity and the measurement of antibody titers. A 14-year-old boy was admitted to our emergency center because of abdominal injury. While he was playing basketball, he was struck directly in the abdomen by the knee of another player. He was diagnosed with ultrasonography and computed tomography of the abdomen as having splenic injury (Type IIIc), and emergency surgery was performed. Because intraoperative gross examination of the spleen revealed a complicated parenchyma breakdown in both the hilum lienis and germinal pole, a splenectomy and autotransplantation of splenic tissue were performed. Thirteen days after the injury, the patient was discharged without any complications. Lienal scintigraphy obtained three months after the injury demonstrated an increase of accumulation at the site of the splenic autotransplantation. The accumulation seen on the scintigram was accompanied with the normalization of IgG and IgM levels, revealing that the autografted splenic tissue had survived and had some level of function.
