Summary:Spherical carbonaceous adsorbent for oral use (AST-120) has been applied to treat patients with liver failure, especially liver cirrhosis, accompanied by encephalopathy and/or pruritus. After administering the absorbent at dose of 5-10 g daily, clinical course, and effect on liver function and bile acid metabolism were examined. Continuous treatment for 3 months or more reduced frequency of encephalopathy, together with reduction of serum ammonia. Abnormally high levels of serum bile acids and low C/CDC ratio were observed in most of patients. The levels were markedly reduced upon the treatment and also predominant reduction of chenodeoxycholic acids resulted in elevation of C/CDC ratio. There were also several cases where pruritus was reduced after the treatment for 1 month or more. Noticeable side effect was not observed even after long term administration. From the results, the adsorbent treatment was considered to be effective to remove intestinal toxic substances which could not be tolerated by impared liver, leading to the improvement of liver disorder.
