Summary:A 48-year-old man with psoriatic arthropathy complicated with clinical feature of ankylosing spondylitis is reported. On admission, severe squamous and erythematous plaques partly with pustules were scattered on almost entire body. All nails were hypertrophic, clowdy and rough-surfaced. A painful swelling of MP joint of the right 1st finger was noticed. CRP was weakly positive, but rheumatoid factor was negative. He had A2, A26, B46, B51, Cw1, Cw3, DR8 and DR6 of HLA. A roentgenogram showed destruction of all proximal phalanges and destruction of MP joint of the right 1st finger. Abnormal uptake of bone scintigram was noted in sacroiliac articulation and lower lumbar vertebras. After all, the back pain appeared and a roentgenogram showed bamboo-spine and sacroiliac arthropathy. Then we diagnosed this case as arthropathic psoriasis associated with ankylosing spondylitis.
