Summary:A 25-year-old man visited an emergency outpatient clinic with the chief complaint of loin pain and nausea after running to a train station at full speed. He was diagnosed with acute renal failure with severe loin pain and patchy renal ischemia after anaerobic exercise (ALPE). ALPE predominantly affects young men. Patients with ALPE present with severe loin pain and nausea. ALPE causes acute kidney injury, which may necessitate hemodialysis. In patients presenting with acute kidney injury with lumbar back pain, it is important to recall ALPE by listening to anoxic exercise such as an all-out sprint during a medical interview.
