Summary:Acromioclavicular joint (ACJ) dislocation is a common injury, often affecting young adults. Complete disruption of the ACJ (Rockwood IV and V) is an indication for operative care, especially in active people. A variety of arthroscopic techniques are used to treat such lesions, however, we will elaborate on one case that was operated in our department, using the TightRope ® System (Arthrex ® , Naples, Florida). This fixation system is composed of a no.5 Fiber wire suture that is tensioned and secured at both ends by metallic buttons against the cortices of the clavicle and the coracoid process. The advantages of this system are: anatomic reduction, permanent stabilization, excellent cosmetics, less morbidity, avoidance of complications from migration of metal implants and last, but not least, the hassle of implant removal.
