Summary:Medical Audit of hospital records has been a major component of quality of care assessment1. The aim of the present Medical audit is to identify the areas for improvement and make necessary changes to provide a better quality medical care to the patients in tertiary care hospital like Nizam's Institute of Medical Sciences, India. The present study reveals that most of medical records were deficient in some or other information. The reasons for the insufficiency of medical records were explained by the doctors that the medical records were filled by naïve resident doctors and also due to heavy workload in areas like emergency department and critical care units preventing them from concentrating in filling the medical records.
