Summary:Benign episodic unilateral mydriasis is a descriptive situation with recurrent unilateral mydriasis in adult people especially women with migraine. A 20 year-old man who presented with paroxysmal left pupil mydriasis and diagnosed as benign episodic unilateral mydriasis after the examinations to exclude the other reasons of anisocoria was reported. In such cases to keep in mind the benign causes of mydriasis would be helpful to avoid unnecessary invasive tests
