Objective To estimate the seroprevalence of human leptospirosis in southern Vietnam.
Methods All pupils (n = 961) of two primary schools in two communes in southern Vietnam were screened for the presence of serum Leptospira immunoglobulin (Ig)G. Leptospira IgM was tested in 92 randomly selected samples. IgM and IgG were tested with a commercially available enzyme‐linked immunosorbent assay (ELISA).
Results Leptospira IgG was found in 123 (12.8%, 95% CI: 10.8–15.1) children with a male:female ratio of 1.5:1. At the age of 7 years the prevalence was 11%. Log‐linear binary regression of the IgG seroprevalence by age showed a mean seroconversion rate of 1.5% (95% CI: 1.23–1.75) per year. Five (5.4%, 95% CI: 2.3–12.1) samples were definitely positive for IgM. Presence of serum Leptospira IgG antibodies was significantly associated with swimming in rivers (RR: 1.745, 95% CI: 1.117–2.724) and wading through water (RR 1.793, 95% CI: 1.181–2.722).
Conclusion Our data emphasize the importance of leptospirosis among children in Vietnam, despite the absence of severe disease, and stress the need for adequate and cheap diagnostics.
Objectif Estimer la séroprévalence de la leptospirose humaine dans le sud du Vietnam.
Méthode Tous les écoliers (n = 961) de deux écoles primaires dans deux communes du sud du Vietnam ont été criblés pour la présence d'IgG sériques de Leptospira. 92 échantillons sélectionnés aléatoirement ont été testés pour les IgM de Leptospira. Un test ELISA a été utilisé pour le test des IgG et des IgM.
Résultats Les IgG de Leptospira ont été détectés chez 123 enfants (12,8%, IC95%: 10,8–15,1) avec un rapport garçons/filles de 1,5/1. A l'age de 7 ans, la prévalence était de 11%. La régression binaire Log‐linéaire de la séroprévalence IgG par âge donnait un taux moyen de séroconversion de 1,5% par an (IC95%: 1,23–1,75). Cinq échantillons (5,4%, IC95%: 2,3–12,1) étaient bien positives pour les IgM. La présence d'anticorps IgG sériques de Leptospiraétait significativement associée à la natation dans les rivières (RR 1,745; IC95%: 1,117–2.724) et le passage à travers l'eau (RR 1,793; IC95% 1,181–2,722).
Conclusions Nos données soulignent l'importance de la leptospirose parmi les enfants au Vietnam, malgré l'absence de maladie sévère, et le besoin de méthodes de diagnostic adéquates et bon marché.
Objetivo Estimar la seroprevalencia de leptoespirosis humana en el sur de Vietnam.
Métodos Se cribaron todos los alumnos de dos escuelas primarias (n = 961) en dos comunas del sur de Vietnam, con el fin de detectar la presencia de IgG anti‐Leptospira en suero. Se buscó IgM anti‐Leptospira en 92 muestras escogidas al azar. Tanto el IgM como el IgG se determinaron mediante pruebas de inmunoanálisis ligado a enzimas (ELISA) disponibles comercialmente.
Resultados Se encontró IgG anti‐Leptospira en 123 (12.8%, 95% CI: 10.8–15.1) niños con una proporción niño:niña de 1.5:1. A la edad de 7 años, la prevalencia era del 11%. La regresión logística binaria de la seroprevalencia de IgG por edad, mostraba una tasa media de seroconversión de 1.5% (95% IC 1.23–1.75) por año. Cinco (5.4%, 95% CI: 2.3–12.1) fueron definitivamente positivas para IgM. La presencia anticuerpos IgG anti‐Leptospira en suero estaba significativamente asociada al nadar en ríos (RR 1.745, 95% IC = 1.117–2.724) y al vadear a través del agua (RR 1.793, 95% IC = 1.181–2.722).
Conclusión Nuestros datos hacen hincapié en la importancia de la leptoespirosis en niños de Vietnam, a pesar de la ausencia de enfermedad severa, y hace hincapié en la necesidad de tener métodos diagnósticos adecuados y baratos. |
