Summary:Highlights • Our case the difficulty of differentiating severe quadriceps contusion from thigh compartment syndrome. • We ed the response of thigh musculature to blunt trauma. • Clinicians should be aware of vascular injury as a cause of thigh compartment syndrome in sports trauma. Vascular examination is warranted in all cases of quadriceps injury. • MRI findings of deep thigh muscle swelling and “blow-out” tear of the vastus lateralis are strongly suggestive of severe quadriceps injury, and may be a harbinger of delayed thigh compartment syndrome.
