Tailored education program using home floor plans for falls prevention in discharged older patients: A pilot randomized controlled trial

Highlights • We conducted a tailored education program to prevent falls in discharged patients. • No falls occurred in the intervention group during the follow-up. • Tailored education had effect of preventing near-falls following discharge.
