Summary:Abstract Background: Cardiovascular diseases are predicted to overtake cancer as the leading cause of mortality in the upcoming few years across the globe. Coronary computed tomography angiography (CCTA) has been designed as a very efficient noninvasive method for assessing coronary arteries in individuals with low to moderate risk of developing obstructive coronary artery disease (CAD). This study was carried out to assess the role of the CCTA-based CAD-reporting and data system (RADS) scoring system in the evaluation of patients presenting with chest pain. Materials and Methods: All patients referred to the department of radiodiagnosis for CCTA with complaints of chest pain fitting the inclusion criteria were included in this study. The CCTA was carried out in 80 patients using appropriate steps. The coronary artery calcium (CAC) score was derived using noncontrast computed tomography (CT). Employing the coronary extraction software, an image of the coronary arteries was generated to evaluate patency, any luminal stenosis, and plaque characteristics. Once the CAD-RADS category was allotted, recommendations were made to the referring physician which helped decide the clinical management of the patient. Results: In the current study, it was noted, with a progressive increase in the category of CAD there was rise in the calcium score and it was found to be statistically significant using the Kruskal-Wallis test with a P < 0.001. It showed a significant correlation between CAD-RADS scores and calcium scores. A significant correlation was obtained between calcium scores and maximal stenosis. Conclusion: CT angiography-based CAD-RADS can help us avoid invasive testing in patients presenting with chest pain, CAD-RADS along with the calcium score is, therefore, an excellent option to assess the burden of CAD.
