Summary:The authors raise several important considerations with the use of high flow nasal cannula (HFNC), including the risk of delayed intubation and higher mortality, strategies for predicting failure, and need for close monitoring. After a patient is placed on HFNC, clinicians should closely evaluate hemodynamic status (heart rate, blood pressure), respiratory rate, and oxygenation status. Laboratory values and clinical scores should never replace bedside clinical decision-making, but rather be used to supplement these decisions at the bedside when considering the need for escalation of respiratory support.Meetings None.Grants/financial support None.Acknowledgements BL, SYL, and SL conceived the idea for this manuscript and contributed substantially to the writing and editing of the review.
