Summary:To the Editor: Tiacci et al. (June 16 issue) 1 report that the BRAF V600E mutation was present in all patients with hairy-cell leukemia (HCL) in their study. However, these findings should be interpreted with caution. Surprisingly, white-cell counts were elevated (>10,000 per cubic millimeter) in 12 of the 48 patients, even though HCL is usually characterized by progressive pancytopenia. Moreover, immunostaining with the use of antiannexin A1 antibody, which is highly specific for HCL, 2 was not performed in 27 patients.
