Summary:Anatomical variations in the distal insertion of the abductor pollicis longus (APL) muscle onto the 1st metacarpal bone and around the 1st carpometacarpal joint led Eduardo Zancolli to introduce a surgical yet conservative partial APL tenotomy targeting its accessory or supernumerary distal bundles. Following an anatomical and pathophysiological review, we will describe the surgical technique for this minimally invasive extra-capsular procedure. Results and operative indications of this very simple technique are reported as well, which can be performed in Dell stage I or II of trapeziometacarpal osteoarthritis when well-conducted conservative medical treatment has failed to achieve pain relief. La variabilité d’insertion de l’abductor pollicis longus (APL) sur la base du premier métacarpien et autour de l’articulation trapézo-métacarpienne a conduit Edouardo Zancolli à proposer une intervention de ténotomie d’affaiblissement de l’APL et surtout de ses faisceaux accessoires ou surnuméraires. Après un rappel anatomique et physiopathologique, la technique chirurgicale de cette intervention extra-articulaire simple est décrite. Ses résultats et ses indications sont rapportés, elle peut être envisagée dans les stades I ou II de Dell d’une rhizarthrose après échec d’un traitement médical conservateur bien conduit.
