Summary:Ten cases of microscopically proved hepatic and lung metastases from epidermoid carcinoma of the anal canal are presented. Microscopic evidence of vessel invasion of the primary tumor and metastatic involvement of the liver and lung are demonstrated. A review of the literature and discussion of the importance of hematogenous metastases in relation to treatment methods are presented. Since hematogenous metastases occurred after abdominoperineal resections for anal cancer (nine cases), the possibility of dissemination at surgery is considered; it is recommended that high ligation of the inferior mesenteric vein be carried out early in the operative procedure.
