Summary:The purpose of this paper is to describe some anatomical variations of the atrial septum that may limit the size of a defect produced by balloon septostomy. The atrial septa of 37 hearts of infants dying of cardiac malformations amenable to balloon septostomy were examined. In five the fossa ovalis was quite small, in 14 the valve of the foramen ovale was thick, and in one the foramen ovale was prematurely closed. These factors may preclude the creation of adequate interatrial communication by balloon septostomy.
