Summary:The aim of the present study was to retrospectively analyze rectal cancer patients and to determine the indication for lateral lymph node dissetion. Curative lateral lymph node dissection was performed on 207 patients from 1970 to 1988. Lateral lymph node metastasis was found in 30 of subjects (14.4%). The important factors influencing lateral lymph node metastasis were as follows : age (p<0.05), division of primary tumor (p<0.05), up ward lymph node metastasis (p<0.01), the depth of invasion (p<0.01), tumor size (p=NS), and tumor histology (p=NS). Lateral lymph node dissection reduced the rate of local recurrence, but patiens with lateral lymph node metastasis had a high incidence of metastasis and poor prognosis. The following factors seemed to indicate lateral lymph node dissection for rectal lymph node dissecion for rectal cancer : 1) Rb and P disease, 2) tumor>3.0cm in diameter, and 3) ss (a1), se (a2), and si (ai) disease.
