Summary:The prevalence of H. pylori infection has been decreasing in Japan (4), and mass screening using endoscopy will become less cost-effective and a greater burden due to low PPV and high false positivity. Because the incidence of gastric cancer was different according to H. pylori infection and H. pylori-associated gastritis, introduction of the ABC method using H. pylori antibody and pepsinogen is considered useful for the identification of a high-risk group of gastric cancer, which can lead to cost-efficient endoscopy screening and also reduce potential harm in a low-risk population (5). See PDF] The association between endoscopy technologies and public health views on disease prevalence resembles that between fishing equipment and fish finder. Changing trends in the prevalence of H. pylori infection in Japan (1908-2003): A systematic review and meta-regression analysis of 170,752 individuals.
