Summary:Moyamoya disease is an unusual disorder of the cerebral vasculature, characterized by internal carotid artery occlusions and formation of a collateral network of the intracranial vessels, which resembles a "puff of smoke" ("moyamoya" in Japanese) on cerebral angiography. This condition has a higher-than-usual association with intracranial aneurysms. We present a case of recurrent intracranial hemorrhage in an adult with moyamoya-like vasculopathy and a basilar tip aneurysm.
