Summary:This study investigated the reasonable range of lymphadenectomy for colorectal carcinoma invading the muscularis propria (colorectal mp carcinoma). Two hundred and eighteen patients with colorectal mp carcinoma were operated on at Keio University Hospital, and the correlation between the extent of lymph node dissection and the prognosis (recurrence rate and 5-year survival rate) was evaluated. According to the operation records, lymphadenectomy was categorized as follows; D3 was lymph node dissection up to n3, D2 was other procedures except D3; L+ was lateral lymph node dissection of #262, #272 and #282, and L-was other procedures except L+. There was no significant difference in the recurrence rate and survival rate between D2 and D3 in patients with colorectal mp carcinoma after curative resection. In addition, there was no significant difference in the prognosis of lower rectal mp carcinoma between L+ and L-after curative resection. From these results, D2 lymph node dissection appears sufficient for colorectal mp carcinoma and lateral lymph node dissection is not necessary for lower rectal mp carcinoma.
